This authorization is a continuing one and shall remain in full force and effect until revoked by the undersigned, or an authorized person on his behalf, by written notice given to Commerce, Attention: Director of Compliance. Such revocation shall become effective only upon the actual receipt thereof by Commerce but shall not affect any liability in any way

resulting from transactions initiated prior to its receipt. This authorization shall inure to the benefit of Commerce, its

successors and assigns. The provisions hereof shall be in addition to and in no way shall it limit or restrict any right that Commerce may have under any agreement with the undersigned. [abstract] objective to evaluate the effect of pullthrough combined with double stapling technique for sphincter preservation operation in low rectal carcinoma. methods pullthrough combined with double stapling technique for sphincter preservation operation was performed on 25 patients with low rectal carcinoma form may 2004 to december 2007. the clinical data were analyzed retrospectively. results fecal continence was preserved successfully in all patients. there was no operative death. only one case got anastomosis leak and 2 cases had incision infection after operation. patients were followedup for 13 years, and two patients had local recurrences. conclusion pullthrough combined with double stapling technique for sphincter preservation operation is simple, safe and feasible for low rectal carcinoma, and enhance the quality of life.

我國直腸癌的特點是低位即距齒狀線5 cm以內、腹膜返折下的癌多達70%～75%[1]。WWW.133229.cOM直腸癌是人類最常見的惡性腫瘤之一，近年來發病率呈上升趨勢。隨著人們對生活質量要求越來越高，需做永久性人工的低位直腸癌患者越來越渴望保留。吻合器的臨床應用使保肛指征更加廣泛。我院 2004年5月至2007年12月采用經拖出雙吻合器保肛手術治療低位直腸癌患者25例，取得滿意效果。現報告如下。

1 臨床資料

1.1 一般資料 本組25例中,男18例，女7例；年齡32～73歲，平均52.5歲。腫瘤距齒狀線3～5 cm。所有患者術前均獲得病理確診。組織學分型:高分化腺癌12例，中分化腺癌11例，低分化腺癌2例。術前進行輔助檢查排除遠處轉移，并行直腸腔內超聲、ct、mr檢查，明確腫瘤未侵犯肛管及未穿出肌層，腫瘤直徑<3 cm。

1.2 手術方法 膀胱截石位，取下腹正中切口。常規切開腹壁各層后，游離直腸上動靜脈，動脈內推注5fu 500 mg后，切斷并結扎直腸上血管。按無瘤操作技術，結扎近端腸管。行直腸系膜全切除(tme)手術，手術從左側游離乙狀結腸，解剖出腸系膜下動靜脈，靠近根部切斷結扎并完成腸系膜淋巴結清掃 。直視下用電刀在前盆筋膜壁層與臟層間銳性分離直至肛提肌平面，保證直腸及其全部脂肪系膜組織被完整切除，下端至肛提肌平面，保護系膜臟層不破損，以防癌細胞播散、種植和殘留。在處理直腸側韌帶時靠近盆壁銳性分離，注意保留自主神經叢。完成腫瘤腸段游離后，距腫瘤10 cm切斷乙狀結腸。擴張至可容納4指后，將切斷的乙狀結腸、直腸(包括腫瘤)從直腸腸腔內套疊拖拉外翻至外，顯露腫瘤及齒狀線。使用稀釋的碘伏溶液反復沖洗拖出的直腸遠端，無張力下距腫瘤遠端2 cm處用閉合器關閉遠端直腸，切除乙狀結腸、直腸及其腫瘤，移除標本。術中送直腸下切緣進行快速冰凍病理檢查，證實無腫瘤殘留時，回納復位直腸的殘端至盆腔。用管狀吻合器在盆腔內吻合乙狀結腸與直腸,吻合完畢后，用腸鉗夾閉吻合口上方的乙狀結腸，盆腔內注入沖洗的生理鹽水并浸過吻合口。經注入氣體，觀察吻合口處有無氣泡冒出，如有氣泡，則應加針縫合。于骶前放置乳膠引流管，自旁戳孔引出并妥善固定。

1.3 結果 本組25例吻合過程順利，全組無手術死亡病例。術后發生切口感染2例，吻合口漏1例。經過保守治療后痊愈。術后1～3個月，患者排便能力正常(大便2～3次/d，成形便)，隨訪1～3年，有2例局部復發，再次行miles手術。

2 討 論

2.1 直腸癌手術切除的理論依據 低位直腸癌的定義是指距齒狀線5 cm以內的直腸惡性腫瘤。低位直腸癌保肛的前提是保證腫瘤遠端直腸切緣無癌組織殘留。低位直腸癌的淋巴引流主要向上，極少向下逆行擴散，僅在晚期、高度惡性、向上引流的淋巴管被癌栓堵塞后才會向下逆行擴散，且其擴散的范圍是極其有限的,很少超過2 cm。逆行擴散>2 cm的病例，均屬分化不良的高度惡性病變[2] 。ruile等[3]報道miles術與低位保肛術的局部復發率分別為21%與28% 。直腸癌的病理、生物學行為研究表明，切除遠端腸管的平面距腫瘤下緣約2 cm，則不會構成腫瘤復發的因素[2]。大宗病例研究已證實，切除遠側腸管2 cm與切除5 cm者的術后局部復發率、生存率等差異均無統計學意義。一般情況下，切除長度只要大于2 cm即已滿足根治要求[3,4]。對于惡性程度不高的直腸癌，遠端直腸切除不少于2 cm是安全可靠的。因為直腸是緊貼于骶骨凹上形成彎曲，術中游離直腸達盆底后，直腸可延伸3～5 cm，從而使部分術前看似不能保肛者得以保留。但是否保肛主要是依據患者的全身情況、腫瘤的分化程度、浸潤轉移的范圍以及距齒狀線的距離而個體化對待。

2.2 低位直腸癌經拖出吻合手術操作要點與并發癥預防 ① 嚴格遵守tme手術操作原則，直視下在骶前間隙、盆筋膜臟層與壁層間行銳性分離；保持盆筋膜臟層與壁層的完整無損，分離時切勿進到直腸系膜內；直腸分離要超出尾骨尖，必須切斷直腸骶骨筋膜，切斷肛尾韌帶。必要時擴大直腸遠端分離范圍，使直腸充分游離，正確估計腫瘤下緣與肛提肌的距離。② 保護輸尿管及盆部重要脈管。由于晚期腫瘤很容易粘連輸尿管及盆部重要脈管，故術中我們常規暴露輸尿管及髂外動脈，以免意外損傷。③ 吻合時應選擇偏大的吻合器，防止吻合口狹窄。④ 充分擴肛至可容納4指，閉合器夾在腫瘤下緣下2 cm處，緊貼閉合器切斷腸管，防止殘緣過長、翻卷入釘座的腸壁過厚、擊發不全出現吻合口漏。⑤ 清除結腸殘端腸壁外0.5～1 cm內的脂肪組織，避免吻合口周圍組織嵌入過多，影響吻合口愈合或術后吻合口狹窄。⑥ 吻合器擊發前一定要檢查無周圍組織夾入，腸管不扭曲，力爭做到一次徹底，擊發后停留約10 s，壓迫殘緣以防出血。⑦ 吻合完畢退出時要緩慢、輕柔、切忌暴力，退出后檢查切緣圈是否完整，如不完整應及時修補吻合口。本組吻合口漏1例，經過保守治療后痊愈。

2.3 直腸癌保肛手術的適應證 實施直腸癌雙吻合器技術保肛手術時，首先強調癌腫的根治性，同時亦要求保存功能或恢復到自然狀態。應掌握以下手術適應證[5]：① 高、中分化腺癌，癌腫距肛緣5 cm以下(或距肛管直腸環2 cm以上)，癌腫未侵犯腸壁漿肌層，或雖侵犯漿肌層而癌腫周圍能切除干凈者，腫瘤直徑<肛管直徑。② 分化差的癌腫如低分化、未分化、黏液腺癌等。侵犯腸壁周徑1/3以下，距肛緣7 cm以上，腸管周圍可切除干凈者。 ③ 無前列腺、陰道、膀胱浸潤者。④ 癌腫雖有腹腔內其他部位轉移，而局部符合上述條件，行姑息性原發病灶切除時，亦可行保肛手術。

2.4 經拖出雙吻合器保肛術治療低位直腸癌優點 ①切斷的乙狀結腸、直腸(包括腫瘤)從直腸腸腔內套疊拖拉外翻至外。有助于確定腫瘤的遠切端以確保獲得充分的腫瘤切緣。經肛管拖出后，克服了盆腔狹小，手術操作困難的缺點，可使病變腸段黏膜在直視下得以徹底清洗從而避免腫瘤細胞的污染和播散。②吻合器應用于低位直腸癌患者或肥胖、骨盆狹小的患者，縮短了手術時間，提高了低位直腸癌做前切除的機會。③采用雙吻合器吻合，吻合口漏發生率也低于經手工吻合者。④因為吻合在盆腔內進行，大多可避免松解結腸脾曲，簡化了手術操作，減少了手術創傷。⑤保留括約肌，保留了的排便功能，避免了低位直腸癌永久性人造給患者帶來的痛苦 。在根治的前提下，患者的生存質量有了很大的提高。

鑒于：

委托人擬在中華人民共和國上海外高橋保稅區設立一外商獨資貿易公司（下稱“貿易公司”），為此，委托人特委托受委托人辦理與設立該貿易公司有關的一切手續和事宜，包括但不僅限于：

（1 ）代為領取有關設立該貿易公司的表格及相關資料，填寫該等表格，起草、簽署和提交有關的申請報告、文件及政府部門要求提交的所有其他相關文件，向有關政府部門提交該等表格、文件、資料和證件；

（2 ）代為辦理外商投資項目名稱預先核準、報批、工商登記及其他相關登記、批準等手續，并代為領取有關的批復、批準證書和營業執照等；

（3）為設立該貿易公司之目的，代為辦理和處理其他任何必要的手續和事宜。

為上述（1 ）至（4 ）之目的，受委托人有權在必要時對其認為勝任的第三方予以轉委托。

本委托書自本人簽字之時起生效，自委托事項全部完成之時終止。
